Instructions for Completing the MISO Survey 2009 Decisions Spreadsheet

Use this form to customize MISO survey items and select optional items for inclusion in the survey.

Submitting customizations
Survey item customizations enable you to refer to a service using the name given to it at your institution.  For example, you might choose to submit a customization that replaces the text “course management system” with “BlackBoard” or “Moodle” in all survey items that reference your institution’s course management system.  To customize an item, enter the text you prefer using the appropriate yellow cell in the Customized text column.  After you enter customized text, the MISO Survey Modification Form will automatically repeat that customization in the gray cells for other survey items that refer to the same service.


Selecting optional items
You may opt to include optional questions in the MISO survey.  For each optional item, begin the selection process by clicking on the appropriate yellow cell in the Optional item column.  Click on the down-facing arrow next to the cell and select either Include or Exclude from the drop-down menu.  You must select one of these options for each optional question.  After you opt to include or exclude an optional item, the MISO Survey Modification Form will automatically repeat that selection in the gray cells for other survey items that refer to the same service.
